THE DIABETES TEAM
Consists of Specialist (Hospital) based multidisciplinary team, and primary health care teams supported by community nursing team.

- Podiatrists
- Gloucestershire Diabetic Eye Screening Service
Services provided from Cheltenham General Hospital West Block Outpatient Department, Tewkesbury Community Hospital and community Podiatry clinics at St Pauls Medical Centre and Hester’s Way Healthy Living Centre. Patient Education sessions are also held at The Leckhampton Surgery.
Additional Specialist services include Cardiology, Vascular, Ophthalmology and Renal.
Primary Care
19 General practices with lead diabetes Practice nurse, GP, community nurses and clerical support.
Services provided in surgeries and some branch surgeries. Domicillary services to housebound patients, residential and nursing home patients.
Patients & Carers
All people with diabetes are part of the diabetes team.
