AIM:
The C&T PCT Diabetes Care Pathway, aims to:
Provide all diabetic patients with the highest standard of care in the most appropriate setting, recognising the principle of informed choice, to control symptoms, and reduce long-term complications.
PRINCIPLES – Underpin the team philosophy of care in Diabetes.
DIABETES TEAM – For Cheltenham and Tewkesbury, includes and involves all professionals involved in the care of diabetics, patients and carer’s. The Diabetes Team crosses organisational boundaries.
CARE PATHWAY – Outlines the organisation of care for newly diagnosed patients, and those undergoing continuing care. Guides patients and professional’s to understand the nature and frequency of follow-up, whether Primary Health Care Team or Specialist Team leads care.
AUDIT – Annual audit of outcomes influences future development of the pathway.
DEVELOPMENT – This pathway will be continually developed by the Diabetes Team, with appropriate organisational changes and investments made to achieve the stated aims of the pathway in line with current expert opinion and National Guidelines.
