Care pathways are developed by doctors, nurses, therapists, social care staff, local managers and, of course, patients in order to work out the best route for patients with specific conditions to receive the most appropriate treatment, care and support.
This Care Pathway has been produced by the Cheltenham and Tewkesbury Diabetes Review Group. It is a good example of care pathways currently in use in Gloucestershire. It is used by local clinicians treating patients with Diabetes. It describes the options and steps along the path from diagnosis to treatment. The aim of developing the Care Pathway is:
"to provide all diabetic patients with the highest standard of care in the most appropriate setting, recognising the principle of informed choice, to control symptoms, and reduce long-term complications."
